With per-thread timers disabled, cyg_thread_delay() does absolutely
nothing.

Are you sure you're calling cyg_thread_delay with the correct units? It's
clock ticks, not seconds/milliseconds/etc.

Have you used or looked at the twothreads example program? That uses
cyg_thread_delay.
