libstdc++-v3/doc/html/manual/policy_based_data_structures_test.html
has this:

In particular, that DTD has:

i.e. every <img> element is required to have an
  alt=""
attribute to be valid according to the DTD.

There are numerous img elements; none of them have alt attributes.

Is this file autogenerated from XML?  I'm not quite sure where the issue is.
