libgomp: Fix reverse-offload for GOMP_MAP_TO_PSET

libgomp/
	* target.c (gomp_target_rev): Dereference ptr
	to get device address.
	* libgomp.fortran/reverse-offload-5.f90: Add test
	for unallocated allocatable.
