Remove references to ada_name_for_lookup (deleted)

This function was deleted on 2017-11-08, but its declaration and
a reference to it in a comment was left behind.  This patch just
removes those.

gdb/ChangeLog:

        * ada-lang.c (name_match_type_from_name): Remove reference to
        ada_name_for_lookup in function's documentation.
        * ada-lang.h (ada_name_for_lookup): Delete declaration.

Tested by rebuilding GDB.
