netdb.h defines IPPORT_RESERVED if __USE_XOPEN2K.  That's the point at which it
entered base POSIX, but it was also in UNIX98 and XPG4, so should be defined
for __USE_XOPEN_EXTENDED as well.  Testing a patch.
