Log message:
	* mkdb.sh: Change to use gnatsd.user_access as user access file
	name.
	- mkdb.sh (domkdir): Provide -p parameter to mkdir, so that
	non-existing parent directories are automatically created.  Fixes
	PR 316.  Both fixes by Pankaj K. Garg.
