If you're adding a completely new target (as in backend), you'll need to add it to gdb. The gdb user's manual talks about adding new ports (and there's a mailing list, too: gdb@sources.redhat.com). If you've already got a port, but it uses its own communications protocol and you simply want to make insight aware of it (so that it appears in the target dialog), edit targetselection.itb, where the target database is stored.
