Funny enough, I was going to suggest something like this as well. Allowing users to _create_ an account, but mute that account so that it either needs to be unlocked (request sent to a list or via a tool) or, even better, have a moderation approach such as you suggest. Bonus points if the number of moderated actions can be configured, so that it becomes a bit harder than just posting a single comment somewhere.
