    staprun: conditionalize use of pthread_mutex*
    
    staprun is compiled single-threaded, but recent improvements to
    util.cxx made it implicitly pthreads-dependent.  Conditionalize
    pthread_* calls with #ifndef SINGLE_THREADED throughout.
