oops, involving utrace_report_death. Possible shutdown synchronization /
module-unload problem.
Following on from bug #13539, we have more work to do.
While the failure is harder to reproduce, oops like the
attached are possible, after hours of concurrent loads.
The problem goes at least as far back as release 1.6,
and likely farther.
