* lisp.c, lisp.h (choose_from_weighted_list): Add argument to
allow "flat" lists of weights and values.
* mknames.c (gen_from_rule): Use it.
* combat.c, mknames.c, mkunits.c: Update callers.
* warn.g: Add tests for namer warnings.
