	Report LOS blockage understandably.
	* history.def (fire-blocked): New hevt type.
	* nlang.c (action_result_desc): Describe it.
	(report_combat_special): Make static.
	* combat.c (check_fire_at_action, check_fire_into_action): Report
	it.
	(check_attack_action): Report A_ANY_NO_MATERIAL if no ammo.
	(report_damage): Don't report actions by dead units.
