* exceptions.cc (call_signal_handler): Zero initialize context and set
context flags, as RlCaptureContext doesn't.

* exceptions.cc (call_signal_handler): Only bother to construct
the ucontext for signal handlers with SA_SIGINFO set. Set
mcontext.oldmask.
