Try
        perl ./collateindex.pl -g  -N -o index.sgml
        db2html -j "-V html-index" main-file.sgml
        perl ./collateindex.pl -g -o index.sgml main-file/HTML.index (*)
        db2html main-file.sgml

(*) the main-file/HTML.index might be places in another 
sub-dir depending on the id you give the book.
