RISC-V: Remove duplicate `#include "riscv-vector-switch.def"`
Hi,
This patch remove the duplicate `#include "riscv-vector-switch.def"` statement
and add #undef for ENTRY and TUPLE_ENTRY macros later.

This patch remove the duplicate `#include "riscv-vector-switch.def"` statement
