Indeed, it is deliberate, since hash_map is a non standard
container: it's a quite useful addition but as such
does *not* appear in the ISO/ANSI standard for C++. I will
look into the insert_iterator bits, however...

hash_map<> being moved to __gnu_cxx:: is a deliberate decision.
The specialization of insert_iterator<> for __gnu_cxx::hash_map<>
being defined in std:: is deliberate because that is where the primary
template insert_iterator<> is defined.
