Currently gfortran and even iFort (Intel Fortran) lowercase all the identifiers.
GDB uses:
    case_sensitivity = current_language->la_case_sensitivity;
const struct language_defn f_language_defn =
  case_sensitive_off,
and in such case lowercases the string in lookup_symbol_in_language.

While iFort lowercases identifiers it explicitly uppercases the module names:

This way module-fully-qualified-name "`modmany`symbol" can never be looked up on
iFort produced inferior (this module support is still not in FSF GDB HEAD).

DWARF3 supports: DW_AT_identifier_case with DW_ID_case_insensitive (+others)
None of GDB, gfortran or iFort support it.

Proper DW_AT_identifier_case would enable gfortran to preserve source file
identifiers case.
