Log message:
cleaning up inline: made configure.in test strictly for "extern inline"
as understood by GCC. Only modifications to header files are to clean up
existing usage of HAVE_INLINE and make it all consistent. No additional #ifdefs
for __GNUC__ at this stage, rely on configure to detect its presence instead.
