    Switch over to version 2 sdt.h.
    
    * dtrace.in (def_append):  Make STAP_SDT_V2 the default.
    * sdt.h:  Likewise.
    * sdt_misc.exp: Likewise.
    * tapsets.cxx (sdt_uprobe_var_expanding_visitor):  Add need_debug_info.
    (sdt_query::handle_query_module): Use dwarf for v2 if a variable ref needs it.
